To organize individuals interested in business and concerned citizens into an association with the objective that their voices may be heard effectively in local, state and national affairs affecting the business community. Optional Group Insurance Plans & Benefits UBA Offers.

UBA Members have the knowledge, resources, and networking they need to be more influential and successful in business. However, as a member, you also get access to advantageous optional group insurance programs to improve your health, financial standing and peace of mind. These programs include:

  • ACCIDENT.
    Group Accident Insurance: Many optional supplemental UBA Gap Plans include Group Accident Insurance or Blanket Group Accident Insurance. Group Accident Insurance offers an extra layer of protection against out-of-pocket costs for accident-related medical expenses. Group Accident Medical Expense Insurance provides up to $5,000, $10,000 or $25,000 (depending on the plan) annually with a $100 deductible per accident. Plans that include Group Accident Insurance also include Accidental Death & Dismemberment Insurance. Accidents happen. Make sure you are prepared for the unexpected.   • CRITICAL ILLNESS.
    Group Critical Illness Insurance: For those with a family history of heart-attack, cancer or stroke, Group Critical Illness Insurance is a way to offer protection for members to be able to visit out-of-network providers, pay for out-of-pocket expenses like deductibles and coinsurance with their comprehensive health insurance plan and to pay for experimental treatments that members might want to participate in. Many optional supplemental UBA Gap Plans include Group Critical Illness or a Critical Illness Insurance benefit as part of the Group Hospital Fixed Indemnity Insurance (depending on plan and state).   • EMERGENCY ROOM VISITS.
    Group Hospital Fixed Indemnity Insurance: Sometimes you just need to visit an Emergency Room or a licensed Urgent Care Center when unexpected emergency accident or sickness medical situations arise. Many optional supplemental UBA Gap Plans include the Emergency Room Visit Benefit as part of the Group Hospital Fixed Indemnity Insurance. The Emergency Room Visit Benefit pays $500 per visit up to 10 visits per year for emergency medical expenses when seeking help from a free-standing Emergency Room, in-hospital Emergency Room or a licensed Urgent Care Center.   • DAILY HOSPITAL CONFINEMENT.
    Group Hospital Fixed Indemnity Insurance: When the unexpected hospital stay arises, make sure you are covered for your out-of-pocket medical expenses like deductibles and coinsurance. Augmenting your existing comprehensive health insurance plan, many optional UBA Gap Plans include Daily Hospital Confinement Benefit as part of the Group Hospital Fixed Indemnity Insurance. Daily Hospital Confinement benefit pays $500 per day for days 1-3 of hospital confinement per policy period.   • OUTPATIENT PHYSICIAN VISITS.
    Group Hospital Fixed Indemnity Insurance: Seeing a doctor when you are sick or for regular check-ups is an essential part of staying healthy. Some of the optional supplemental UBA Gap Plans include Outpatient Daily Physician Office Visit Benefit through the Group Hospital Fixed Indemnity Insurance. Members who elect plans that include this featured benefit will have access to $125 per visit with 1 medically necessary visit and 1 visit for wellness per year. Take care of yourself and see a doctor regularly to maintain your health and to be on top of future medical issues.   • OUTPATIENT LABS & X-RAYS.
    Group Hospital Fixed Indemnity Insurance: When you need radiology, an x-ray or need to have labs taken to help diagnose or treat a condition, the Daily Outpatient Diagnostic Radiology & X-ray and Daily Outpatient Diagnostic Laboratory Test Benefits are a way to help. Some of our optional supplemental UBA Gap Plans include both the Daily Outpatient Diagnostic Radiology & X-ray and Daily Outpatient Laboratory Test Benefits as part of the Group Hospital Fixed Indemnity Insurance. These benefits pay $75 per visit with 1 medically necessary visit and 1 visit for wellness per year.   • TERM LIFE.
    Group Term Life Insurance: When it is our time, we want to be able to help our loved ones financially to help offset costs of funerals or just to provide a little something to say I love you. Many of the optional supplemental UBA Gap Plans include Group Term Life Insurance providing either a $5,000 benefit or a $10,000 benefit (depending on the plan).
